# Attach Feedback Template

You must first attach the feedback template to your channel before using it. Go to the **Quality Assurance** menu and then, click the **Feedback Template** page. On the template feedback page, select the template you want to attach, then click the **'Attach'** button.

Then, you will see a pop-up attach feedback template to the channel. Select your objected channel in the available channel section and click the **'plus'** button or you may use the **'Attach All Channel'** button to select all channels. After that, you will see the successful channel attach in the Attached Channel section.

**Note:** Only one feedback template can be used per channel. That channel will be removed from the list of available channels once it has a feedback template.
